If you would like to register your child for Lake Center’s Children’s Program, you can do so at the Registration desk or by using this form.  If this Registration form is completed and submitted by 7:00 a.m. on or before the Sunday that you plan to attend, there will be a Registration Card waiting for you.  Permanent name tags will be provided for you and your children after your third visit, but will be discarded if the child is absent for 12 consecutive weeks.  Please email Amy Powers or Mark VanderKlok if you have any questions.
